Please download and complete the Institute Registration form by either clicking on the link HERE, or the link at the right. This is a PDF document and you will need Adobe Acrobat Reader to view it. Acrobat Reader is a free utility and can be downloaded by clicking on the graphic below.

Registration and Fees
$35 per day or $150 full Institute - Fee includes lunch and course materials. Please send registration form with payment (registration fee ONLY) to:

CANCELLATION POLICY AND REFUND POLICY:
If you are unable to attend and you would like to receive a full refund, you must let us know at least 7 days prior to the start date of the institute.
